Capitol Imaging Services offers the advantage of accessing patient screening and diagnostic exam images and reports through our PACS. To request access to our PACS, please complete the form below.

We ask for an NPI number in order to make sure we are accurately looking up the correct information. We thank you in advance as that helps us in expediting the process.

Once we receive your request, our I.T. department will be happy to assist you. Once your account is activated, you will receive an email from Capitol Imaging Services using the email address you provided in your request.
